Policymakers can strengthen the incentives for parents to send their daughters to school by setting the no-fee policy. However, in economics, nothing is for free. There is always someone or something that pays for it. A no-fee policy may be brought about by taking a portion of the peoples taxes. Another thing the policymakers can do is provide stipend for girls who enroll in girls school. This would surely increase the girls enrolment rate as the graph below indicates that the fall in fees from f to f1 will lead to an increase in the enrolment rate from e to e1. Providing every eligible girl in South Asia and Sub-Sahara Africa would contribute towards the countrys development because major studies of growth conclude that improved knowledge, higher efficiency, and better education play a vital role in the functioning and productivity of an economy. The World Bank has pointed towards human development which they believe better education, and family planning can promote development as effectively as capital investment in physical plant. As the article states that 99% of the 529,000 women who die annually from pregnancy related hurdles come from developing countries and the reason for that is the lack of education for women in those countries. Uneducated women do not know the severe malnutrition and prenatal care during pregnancy which puts both the child and the mother at serious risk. Expenditure on human capital better educated, health, and nutrition tends to reduce birth-rates and infant mortality. The women will be educated and will be able to effectively take part in family planning as she will be aware of the consequences. Having a controlled population means a positive result in the GNP per capita which is the result of subtracting the population growth rate from the GNP growth rate. Educating the girls will privilege them to be able to raise their child effectively in a civilized manner. Educated women will be aware of dangers such as HIV/AIDS, poor sanitary habits and poor dietary habits. So basically they will be aware of the environment around them and above all they will know their rights and therefore fully stand up for themselves. So, educating the girls would not only benefit themselves but the society as a whole as well. So education helps the overall population and on a larger scales the whole world. Providing education to the girls would also add to the countrys labor force which would definitely lead to an increase in economic growth. In my opinion, I would say that providing education to girls in South Asia and sub-Saharan Africa by building single-sex schools is a much appreciated step because of the numerous benefits it will contribute to the world. However it has to be taken into consideration that in some parts of the world providing co-education is unacceptable for example the northern areas of Pakistan. Therefore single-sex education will be a much better choice as it will be acceptable and also appreciated. However, single sex education is said to be not as beneficial as the co-education because in co-education pupils of both gender get to interact with each other and learn much more than they can in single education. But single sex education will be the only choice that can be perused in areas where co-education is not allowed. Although building single-sex schools will have an opportunity cost as the government will have to sacrifice something in order to afford the cost of building schools, but I would say it is all worth it. When the girls of a country are educated they will definitely add to the countrys work force and educating girls means educated women in the country which means that they will know how to properly raise their children and nurture them. There are many benefits which the country will have from providing education to the girl. There are short term benefits as well as long term benefits. More emphasis will be on the long term benefits as you know that it will take time before the women of the country will all be educated and it will take time to have an impact of providing education to girls on countrys women literacy rate. I also found the idea proposed in the article by the policymakers about handing the money to the mother of the school-going girl, instead the men. Handing the money to the women is a very effective and well-thought idea as mothers are more concerned about their childrens health and nutrition.

For at least I know I'm free." Freedom is the founding pillar of the American self- perception. Self-perception is the culmination of how one views oneself. Other aspects which make up American self-perception are wealth, power, and the pursuance of happiness. Self-perceptions, whether confined to the individual or confined to an entire country, usually leave out negative aspects such as hypocrisy. When dealing with the perception of a country, the true image of a society comes from self, or internal perceptions, combined with the external perceptions from other countries. The foundation of American self-perception is freedom. Freedom of speech and movement are virtual institutions in the United States. Such freedoms of speech and movement are outlined in the United States Constitution. Americans believe the constitution sketches the "American Dream" which is having a family, money, and the freedom to pursue happiness. Every American will stand by the line derived from the Constitution, "All men are created equal." In actuality, the constitution outlined the freedom for rich white landowners to achieve unchecked power and wealth. At the time of the framing of the constitution, blacks were slaves thus all men were NOT created equal. Women were equally excluded from the constitution as suffrage wasn't even a consideration at the time. The only class groups which the American Constitution outlined freedom for were wealthy European immigrants fleeing their own land for such reasons as taxes. After such movements as Suffrage and Civil Rights, all Americans were granted individual rights of freedom thus approaching equality. The American self-perception of living a life of virtual complete freedom parallels the American stand on its' belief of democracy. Americans feel that a democratic government is the only possible administration which can be deemed acceptable in today's world system. Nurses now must confront life-and-death decisions and in short periods, even on an hourly basis. Medical advancements in new technology have the equal potential of benefit and harm. Todayââ¬â¢s nurse makes her own decisions, that is, without the influence of her employer. But how she does is under the control of a Code of Ethics to which she swears (Wilkins). In general, nurses are employed by a hospital, clinic or some private entity (Wilkins 2007). Yet seemingly ââ¬Å"independentâ⬠decisions made on patient care not really totally independent. Each decision, especially a critical or end-of-life decision, creates a ripple effect, which extends to At first glance of reading the ANA code of ethics it seemed way beyond my ââ¬Å"scopeâ⬠of practice. I took a lot of time to read since it was kind of repetitive in a sense. Iââ¬â¢m a new graduate to nursing field and without experience I donââ¬â¢t have ample nursing philosophy yet. I have worked as a hospice nurse now for 3 weeks, and have a nursing perspective of 3 weeks. Itââ¬â¢s hard to imagine the code of ethics being realistic, 100% of the time. As nurses we are humans and not robots. The ANA Code of Ethics for Nurses serves as a prototype for me just entering the field. Itââ¬â¢s what I will attempt to stand by. It serves as the framework to what I will strive for. The first three provisions describe the most fundamental values and commitments of the nurse, the next three address boundaries of duty and loyalty, and the last three address aspects of duties beyond individual patient encounters. The first 3 provisions emphasize the fundamentals, commitment, and advocacy of the nursing profession. Acknowledging a someoneââ¬â¢s dignity is an integral component of nursing care. This is Not that I have a whole lot of experience being a nurse but when Iââ¬â¢m able to make the patient feel comfortable, I get a lot more accomplished. Getting the patient to feel comfortable with you makes a huge difference. I try and decrease any awkward barriers between the patient and I . In the short time I have been a hospice nurse, it has already humbled me tremendously. When you are present in the aide for the moment in life where people are so vulnerable, it changes you. It adds to the dynamics you carry as a nurse. One of my patients is a 93 year old female who lives with her husband of 75 years in an ALF. Not any physical co morbidities besides her old age. Upon her assessment she began talking in stated that she was tired of living and that her and her husband fall asleep and pray to not wake up. Which about made me want to cry. I immediately asked why she felt that way, she stated that she has no reason to live, and the ALF gets old and boring. I replied stating that there is a reason for her living, and that everything has a purpose. I of course didnââ¬â¢t know exactly why, didnââ¬â¢t have those kind of answers. I was learning from her that I was grateful and appreciative to experience this time with her. As I was leaving she expressed that I gave her some push to smile and that I made her feel so much better. The The ANA code of ethics sounds crazy to me. It makes me feel like a robot nurse, so repetitive. Realize itââ¬â¢s a set of standards that were to work up and be about. So many elements in what makes a good nurse. At times I feel like how is a possible to be a perfect nurse in every situation. Were human too and can from suffer from burnout and may need to be the patient. The final three provisions examine nurses duties and obligations beyond the general practices associated with caring for individual patients. 1. The nurse, in all professional relationships, practices with compassion and respect for the inherent dignity, worth, and uniqueness of every individual, unrestricted by considerations of social or economic status, personal attributes, or the nature of health problems. 2. The nurses primary commitment is to the patient, whether an individual,family, group, or community. 3. The nurse promotes, advocates for, and strives to protect the health,safety, and rights of the patient . 4. The nurse is responsible and accountable for individual nursing practiceand determines the appropriate delegation of tasks consistent with thenurses obligation to provide optimum patient care. 5. The nurse owes the same duties to self as to others, including theresponsibility to preserve integrity and safety, to maintain competence, andto continue personal and professional growth . 6. The nurse participates in establishing, maintaining, and improving healthcare environments and conditions of employment conducive to the provisionof quality health care and consistent with the values of the professionthrough individual and collective action. 7. The nurse participates in the advancement of the profession throughcontributions to practice, education, administration, and knowledgedevelopment. 8. The nurse collaborates with other health professionals and the public inpromoting community, national, and international efforts to meet healthneeds. 9. The profession of nursing, as represented by associations and theirmembers, is responsible for articulating nursing values, for maintaining theintegrity of the profession and its practice, and for shaping social policy Provisions 1-3 The nursing ethics provisions start at the patient level and progress to a wider sphere of influence. The Important Parts Of Giving A Speech Firstly, the most important part of presenting a speech that cannot be done wrongly is the preparation for the speech which the speaker is about to deliver. There are a few mistakes made by the speaker that obviously shows that he is not prepared for his speech. One of the most obvious mistakes done by him was he did not rehearse his slides before his presentation. He even forgot the title of the speech he is delivering and had to refer back to the slides to see what his title was. As part of his preparation, he should rehearse his slides at least 7 times before he starts presenting his speech. He should stand in front of the mirror and practice presenting his speech so he can analyze what his presentation is lacking. With that tried, he could improve his speech delivery. Based on the text book written by Stephen E. Lucas, he quoted that a powerful speech could have genuine impact on the listeners and as a speaker he should have an ethical responsibility to consider that impact and t o make sure he is fully prepared so as he do not convey misleading information to the audience (Lucas, 2008). Misleading information could have cause tragic result to the audience as they trust the speaker for what they say. No matter what the topic is, the speaker should do extensive research to avoid any misleading or inappropriate information. Besides that, the title of the speech was too broad as well. This shows that he did not narrow down the topic. This part of his preparation is vital, as his topic should not be too broad and should be more focused before delivering to the audience. Otherwise, audience will not pay attention to what he is saying. Another mistake by the speaker was that his goals are not ethically sound. Based on the textbook, a speakers goal should be strong on its ground (Lucas, 2008). In his book, Stephen E. Lucas also mentioned that a responsible public speaker cant escape the ethical soundness of their goals. The speaker should also prepare cue cards so that he can refer and also to remind him about his points when he is delivering his speech. Cue cards should contain the key words which reminds the speakers of their points and explanations. No audiences want to see their speaker stutter on stage, because this shows the lack of confidence and lack of preparations for the speech. Thus, the preparation before a speech is a vital process in giving a speech. Secondly, another important part on presenting a speech is that cannot be done wrongly is the delivery to the audiences. The speaker made a few mistakes on the delivery aspects of his speech. To start off, he pauses frequently when delivering his ideas or thoughts due to the lack of preparations. Therefore, there are many awkward silences in the middle of his speech. Moreover, the speaker always filled the silence with the sound erm ah and so on and this is actually a very crucial mistake for a speaker. A speaker should not do that while delivering a speech because it will give the audience a negative perception or thoughts on that particular speakers intelligence. A good speaker should pauses his speech at the right time which means he should pauses in the end of his thoughts or ideas but not in the middle of it because it will confuse the audience. Secondly, the speakers voice is not clear and he likes to babble when he delivering the speech. These will actually make it difficult f or the audiences to listen to his points. As a result, the audience may not able to get full and precise information from the speaker. Therefore, voice clarity is actually an important factor in order to have an ideal speech delivery. Therefore, the speaker should speak and pronounce the word by word clearly to the audience regardless whether it is slow or fast.(Lucas, 2008) Besides that, another mistake the speaker made is that he did not use transitional devices or connectors in his speech. As a result, this could make the speech delivered ineffective because normally, connectors or transitional devices are used to signal the beginning and the end of the ideas, thoughts and sentence. In addition, the speaker also has problems with his body gestures when he is delivering his speech. To begin with, his body gesture throughout the speech delivery seems to be unnatural. As a result, the audience will feel that the speaker is not well-prepared or confident enough in delivering his spee ch. As we all know, body gestures normally appears naturally because it helps to clarify and reinforce the speakers idea. On the other hand, the speaker has a problem with his eye contact. He seems to not have eye contact with his audiences mainly because he is always looking back at the slides because he is not sure whether which point he should deliver next. Therefore, he loses his eye contact with his audience so he failed to analyze the audiences reactions and feelings while he was delivering his speech. He should prepare well and focus more on his eye contact with the audience because it is crucial to know what the feelings and reactions of the audience are while he is delivering his speech and identify his mistakes if he feels that audience is not comfortable with his speech delivery. Other than that, he is also having problems with his body movement when he gave the speech. From the video, we can see some of the body movement he likes to do. For instance, he likes to put his hand inside his pocket, lean forward, and shift his weight from one foot to another. This actually shows that he was nervous during the speech delivery. Moreover, there is also a disadvantage when a speaker leans forward because when a person leans forward, his head will look down to the floor so it will affect his voice projection. As a result, the voice is not projected straight to the audience instead the voice is projected to the floor. He should instead stand straight up while delivering the speech as to project his sound towards the audiences.( Lucas, 2008) To conclude, the delivery of a speech is a very important criteria. If done wrongly, it could cause mislay and inaccuracy of information. Thirdly, another important part of presenting a speech that cannot be done wrongly is the visual aids used. The mistakes that were made by the speaker while using visual aids can be categorized into two different categories, which is under the preparation of visual aids, and the presentation of visual aids. The first mistake that the speaker made from the category of preparations of visual aids is that the points in PowerPoint slides is not well organized and is complicated. Visual aids should be simple, clear, and straight to the point. Besides that, the points should be narrowed down and limit to a manageable amount of information. The basic rule is to include in your visual aid on only what you need to make your point. The second mistake is that the visual aids are too small for the audiences to see. There is no point for a visual aid if no one can see it. So bear in mind the size of the room in which you will be speaking and make sure your aid is big enough to be seen easily by e veryone. The third mistake is the use of unsuitable fonts. Not all fonts are suitable for visual aids. The speaker should avoid decorative fonts. He should try to use a limited number of fonts. Some variety of fonts in a visual aid is appealing, but can be distracting. (Lucas, 2008) Using two types of fonts as per heading and per content is sufficient for a visual aid as suggested by experts. The second category, presentation of visual aids, under which the speaker made a mistake, is that he actually talks to his visual aid instead of the audience. The speaker should talk to the audience and not to the visual aid. When explaining the visual aid, it is easy to break eye contact with audience and speak to the aid. Of course, the listeners are looking at the aid, and will need to glance at it time after time as the speaker talk. He may lose his audience if u keeps his eyes on his visual aids during the speech. By having eye contact with the audiences he may benefit from the feedbacks b y the audience on the visual aid he is using. Another mistake the speaker made is that the explanation is insufficient, not clear and not concise. Everyone knows visual aids do not explain it selves; the speaker needs to explain his visual aid to the audience. But sadly, all he does is reading from it. It is a fundamental knowledge of giving a speech that explaining the points on a visual aid is a must. The speaker should explain in detail what he included in the visual aid as it is vital information on his speech and it is important for the audience to understand it as it is the whole point of using a visual aid. The speaker should remember that a visual aid is useful only if the explanations come with it. (Lucas, 2008) Therefore, to fully utilize the visual aid, the speaker should avoid making these fundamental mistakes as it will cost him the audiences attention. Last but not least, another important aspect of giving a speech that cannot be done wrongly is the method of usage of the PowerPoint slides. The mistakes that the speaker made when he was using the PowerPoint slides can be divided into two categories, which is the format of the slides, and the delivery of the speech with the slides. The format of the PowerPoint slides consists of color, text, fonts, images, space, and animation of the slides. The first mistake the speaker made in the format of the slides is including too much texts in she slides. He should not include too much text in a single slide; including too much text will cause the font size to decrease, thus causing difficulty in reading for the audiences. He should instead limit himself to a few lines per slide and make it brief. The second mistake the speaker made is that he failed to give enough spaces for the contents in the slides due to the overload of texts per slide. This causes the slide to lack visual balance and co uld be hard for the audiences to read. According to textbook by Lucas, one must make sure that ones slide is provided with adequate amount of visual balance no matter what kind of element was included in a slide.( Lucas, 2008) Subsequently, the speaker also made a number of mistakes in delivering his speech using the PowerPoint slides. As we know, to deliver a speech using the slides needs good timing and careful planning. It will be disastrous if the slides went wrong during the presentation. The first mistake the speaker made in the delivery of the speech with the slides is that he could not find the slide he was looking for during the presentation. This means that he did not recheck his slides after doing them and may have left out some vital information. What he should do is to always double-check his slides for mistakes or in this case, left-out information. (Lucas, 2008) The second mistake that the speaker made is that he did not prepare and rehearse with his slides. This can be seen during which he had trouble linking his hand notes with his slides. The speaker abandoned the notes and read from the PowerPoint slides instead. He should have rehearse with his slides before presenting because using PowerPoint in presentation needs good timing and careful planning. Another mistake the speaker made is that he did not check the room and equipments provided before the speech. Judging by the fact that he did not use a projector to project is slide could mean two scenarios; either the room does not have a projector or the fact that his computers port is not compatible with the projector port. One way or the other, he should have checked the room and equipments and make necessary preparations or adjustment before deciding on doing a PowerPoint presentation. (Lucas, 2008) Thus, the mistakes that the speaker made in using the PowerPoint slides is very fundamental and should be avoided at all cost because it will gravely affect the outcome of the presentation. In conclusion, these four aspects, preparations, delivery, visual aids, and methods of using PowerPoint slides are of utmost importance and shouldnt be taken lightly by the speaker if he wants to give a good speech. Delivering a good speech is not a natural talent but a talent nurtured by the speaker themselves. Everyone can be a good speaker; all you need is sufficient preparation, proper delivery, easy to understand PowerPoint and also attractive visual aids. We believe that if all of these aspects were properly done then the speech delivered by the speaker will be convincing and attracting to the audiences. Kurt Cobain Kurt Donald Cobain was born on February 20,1967 in the town of Aberdeen, Washington. Aberdeen is on the west coast and is about 108 miles southwest of Seattle. Aberdeen is a dreary place with about seven feet of rain a year. Kurt was born to Mrs. Wendy Cobain and to Mr. Donald Cobain. Wendy was a homemaker. She had a very tight bond with Kurt. He was her first born. She had another child three years after she had Kurt. Her name was Kim. Donald was a mechanic. He was very into sports. He often tried to push Kurt into sports but Kurt just didn't like them. Even though the Cobain's didn't have much they seemed to do all right. Wendy dressed her children in the best clothes she could afford. They always looked like the best dressed kids in Aberdeen. Kurt was an extremely happy child. He would wake up everyday so happy. He was always filled with joy and always had a smile on his face. Kurt once said his upbringing could be decribed as "white trash posing as middle class". His mother told him to stay away from the poor kids. She said they were dirty. So Kurt did and he also would beat the up. Then in 4th grade he realized he liked them better. Around that time people started to notice that Kurt was very good in art. Most of Kurt's friends didn't really like things like art and music. He loved these things so much he stopped making friends because he was different. Kurt was not such a health kid. His whole life he suffered chronic bronchitis. At age of seven he was diagnosed hyperactive. He was put on Ritalin. This seemed to make him stay up until four in the morning. They soon put him on sedatives. This did not work either. They made him fall asleep in school. Doctors told Wendy to try subtracting sugar and Red Dye #2 from his food. This was the right prescription for Kurt. Sometime during the 8th grade Kurt was diagnosed with scloiosis. In his later years the weight of his guitar made it worse. Suicide also ran in his family. In 1979 a great uncle committed suicide and in 1984 an uncle also committed suicide. Kurt was in love with music since the age of two. He was brought up in a musical family. One of Wendy's uncle had some records out in the 40's and 50's. Her brother was in a Rock and Roll Band. Her sister was in a country band and played the guitar. Almost everyone had some sort of musical talent. Kurt had always wanted to be a ... ... Kurt wakes twenty hours later and denies it was an attempt. At this time Nirvana decides to cancel the rest of the tour and take a rest. Through all of the tours their are rumors that tensions are high between the group members. The press says there is constant fighting. There is even a rumor that the group is going to break up. To add to the cancellation of their tour they also cancel all their involvement in Lollapalooza. Then on March 18,1994 a startling thing happens. Kurt locks himself in a room of his house. He will not come out and he has a 38 caliber revolver with him. He threaten to kill himself. Courtney Love once again calls the police. The police come and take control of the situation. They take the gun away. Some time around March 30,1994 Courtney calls for an intervention to get Kurt off of drugs. She realizes he really does need help. On March 31,1994 Kurt checks in to the Exodus Recovery Center. It seems as though Kurt wants help. This doesn't last long. The day after he checks in he jumps a wall and it is the last anyone sees of him. He is missing On April 8th an electrician finds Kurt dead in home, from an apparent self- -inflicted gun shot wound to the head.

Multiple Relationships
Multiple relationships, being in not just one relationship but more. Why is it so connected to ethics? For the reason that generally, when we hear it the first time, we most likely to think of infidelity, a betrayal to a partner. And infidelity is related to ethics, opposition that is. à How could we possibly overcome this? We must first recognize what ethics is, and then figure out why people engage in multiple relationships. According to the Markkula Center for Applied Ethics of Santa Clara University, ethics is two things. First, ethics refers to well based standards of right and wrong that prescribe what humans ought to do, usually in terms of rights, obligations, benefits to society, fairness, or specific virtues. Secondly, ethics refers to the study and development of one's ethical standards. These ethical standards pertain to sensible duty to avoid doing criminal acts, to initiate virtues and issues on rights which are sustained by reliable and justifiable grounds. Ethics could also mean that we should stay to live up to what society had strongly built. This means to say that ethics cannot be lock up to religion, for religion could only relate to religious people and anything concerning with religion. Same thing with law which often included these ethical standards that most general public follow yet it can be disagreed to but with punishments to be face. There are reasons why people engage in multiple relationships: they are unsatisfied with their partner, can it by intimacy, passion, commitment; they can no longer get hold of long-distance relationship; they saw somebody better or basically they just fell out of love, and that they cannot afford not to have a companion, that is, they donââ¬â¢t want to be alone. Multiple relationships deviate to ethics when it supposedly mess up oneââ¬â¢s own objectivity, competence, or effectiveness in performing his or her functions in society, or if not risks exploitation or harm the person with whom he or she has relationship with. But no matter how ethics intervenes to this issue, we cannot get away with the fact that influence of media which involves liberal behavior leads to having rational minds and non-judgmental attitude in understanding multiple relationships. We became so open with such possibilities that these things happen without preferences. There are really people who cannot settle with just one relationship, unsatisfied with what they have, and leads to finding and having somebody else to be with. He equates the utility of human beings with that of animals and as such humans have no right to exercise control on the life of animals. That animal cannot reason or talk is no issue for Singer. The point is it suffers. Singerââ¬â¢s argument is based on the solid grounds of scientific approach and it has the element of unassailable logic. He argues that some of the animals are more intelligent than the human babies and/or mentally retarded. Ethical considerations demand that such animals need to be excluded from biomedical research. Moreover, the onus of proving that the benefits of research are in tandem with the suffering and pain imposed on the animals, vests with the scientists. Management of Mandibular Condylar Fractures (MCF) Summary This paper reviews the literature of evidence based studies and textbooks on the management of mandibular condylar fractures (MCF). It also provides a brief outline of the signs and symptoms as well as the classification system of MCF. Optimal management of MCF is a controversial topic and there is also no consensus on its associated classification system. Overview The mandibular condyle is an articular surface that is part of the temporomandibular joint which facilitates the rotational and translational movement of the mandible. The condyle varies in appearance between individuals and also by age group to accommodate developmental variations, and also due to malocclusions, trauma and diseases. The condyle is a weak point due to its relatively low stiffness and as such is more prone to fractures than other parts of the mandible. The condyle acts as a buffer primarily to reduce intracranial injuries. Based on review of two studies, Boffano et al, 2015, reported that condylar fractures ranges from 35% to 43% of all mandibular fractures. According to Afrooz et al, 2015 condyle fractures represent 27.4% of all mandibular fractures. Based on their review of the US National Trauma Data Bank records, they also conclude that mandibular fractures are primarily caused by external causative factors such as assault, motor vehicular accidents and falls. Internal causative factors of fractures can include osteomyelitis and tumors; but are not as significant as the external factors. Signs of Mandibular Condylar Fractures Condylar fractures can be unilateral or bilateral. A patient with condylar fracture can show the following signs (Peterson Kruger, 2011): Occlusal Prematurity Reduction in vertical height of the mandible on the injured side as a result the injured side makes contact first; sometimes followed by rocking motion and then occluding to maximum intercuspation. Careful observation of the mandibular closure will show the alteration in the alignment Inability to Achieve Maximum Intercuspation In bilateral fractures, due to the fracture of both condyles there is a premature contact of the posterior teeth with a large anterior open bite. Bilateral condylar fractures along with symphysis fracture can cause posterior cross bites and anterior open bites Ipsilateral Laterognathia- In unilateral fractures there is laterognathia on the side affected by the condylar injury. Due to retrodisplacement of the mandible on the injured side, there may be Class II molar malocclusion on that side. Ipsilateral Deviation on Opening- In unilateral fractures, while opening the mandible gets deviated to the side of the injury Balancing Side Occlusal Interference during Contralateral Mandibular Translation Balancing side interferences may be present when performing contralateral jaw movements in unilateral condylar fractures Limited Mouth Opening This can be attributed to interferences of the fracture, displacement and/or rotation of the segments, coronoid impingement, bleeding, edema and pain from joint splinting. Classification of MCF There has been a lack of general consensus on the classification of the anatomical mandibular condyles which in turn has caused disagreement on the most effective way of management of MCF. There are a few systems used for classifying Condyle fractures. In the earlier days, a number of classification systems were based on radiological X-rays and were not supported by surgical experiences and findings. These systems included those proposed by Kohler, Reichenbach and Wassmund. The Lindahl system developed in 1977 is a popular method of classifying condylar fractures. In this method, the position of the fracture is the main determinant as to whether it falls in the condyle head, the condyle neck or the condyle base. These areas are delineated by specific landmarks and reference lines. This system was refined further by Loukota et al in 2005 by subdividing the condylar process more precisely and delineating defined anatomical landmarks[1]. However a drawback of this classification method is that the degree of displacement or dislocation which is essential to surgical intervention is not captured. The Spiessl method categorises the condylar fractures according to the point of occurrence of the fracture (low or high) without displacement and with displacement or dislocation as well as condylar head fractures. This classification system has been used in clinical and scientific use however there is still a limitation in that it does not define the degree of angulation nor clearly articulate the borderlines between low or high fractures. Management of MCF The management of mandibular condylar fractures has been a controversial subject. The options to management of condylar fractures include: do nothing (observation), closed reduction or open reduction techniques. With the do nothing option only practical in the simple and straight forward cases, the real debate is whether to pursue closed or open reduction. A number of variables must be taken into account to determine the treatment method. These variables include the patients age, presence of teeth, severity of fracture of the condyle and associated fractures of the mandible, fracture height, extent of malocclusion, patients adaptation, patients masticatory system and unilateral or bilateral occurrence. A literature review of many evidence based studies was undertaken with advantages and disadvantages of both the closed reduction and the open reduction methods as postulated by researchers and clinicians. The summarised findings are outlined below. Eckelt et al, 2006 undertook a study on 66 randomised patients and compared the results of open versus closed treatment of fractures of the mandibular condylar process. They stated that correct anatomical position of the fragments was achieved significantly more often in the operative group in contrast to the closed treatment group. They also found that the patients who had operative treatment reported less pain. In terms of mandibular function impairment, they reported that the persons who had operative treatment had less pain and discomfort. They also reported that there was significant differences in mouth opening/lateral excursion/protrusion between both groups (open 47/16/7mm versus closed 41/13/5mm). They concluded that both treatment options for condylar fractures of the mandible yielded acceptable results. However, operative treatment, irrespective of the method of internal fixation used, was superior in all objective and subjective functional parameters. In a study conducted on 27 patients in India, (Ragupathy, K 2016) comparing the outcomes of surgical vs nonsurgical treatment of mandibular condyle fractures, he reported that no group had malocclusion. Of the 11 persons in the open reduction group one person had a post-operative infection and two had temporary facial nerve weakness. In the closed reduction group, nine patients had loss of vertical ramus height and six had reduced mouth openings (less than 35mm). He concluded that .. nonsurgical treatment gives satisfactory clinical results, though the condyle is not anatomically normal in radiographs, whereas surgical treatment provided more accurate results clinically as well as radiographically. Kysas, 2012 et al undertook a meta -analysis of 20 studies involving analyses of 1,186 patient comparing closed reduction to open reduction in patients with condyle fractures. It must be noted that only 4 out of the 20 studies were randomised control trials (RCT). In addition, they noted that there was significant variation between treatment protocols, follow-up periods, and outcomes measured. Kysas considered a number of post treatment functionalities such as status of the post-treatment occlusion, mouth opening, protrusion, facial height, pain and the presence of postoperative ankyloses. They also considered facial nerve weakness and scarring in the case of open reduction method. The 4 RCT studies reported statistically significant conclusions favouring open reduction method over the closed reduction method; however Kysas et al found some shortcomings in these studies methodologies. Kysas et al concluded based on their meta-analysis that open reduction method for condylar fractures may be as good as or better than closed reduction. In addition, they reported that morbidity associated with surgery is low. However, they caution that available evidence is of poor quality and as such not strong enough to change clinical practice. Choi et al, 2012 summarised the advantages and disadvantages of both open and closed reductions methods. Advantages of Closed Reduction No injuries to nerves or blood vessels. No post-operative complications such as scar or infection. No tooth germ injury occurs because there is no establishment of crown of permanent teeth; this is beneficial for pediatric patients. Disadvantages of Closed Reduction Because of the insufficient reduction of bone fragments, there can be disorderly or excessive growth of the mandible and displacement of the ramus or mandibular deviation can occur. In addition there can be injury to the periodontal tissue and buccal mucosa, poor oral hygiene, pronunciation disorder, imbalanced nutrition, mouth opening disorder, and respiration disorder. Advantages of Open Reduction Minimise the number of displaced bone fragments to the best location possible. Prevent future complications such as respiratory disorder, original pronunciation and minimise nutritional imbalance Disadvantages of Open Reduction Possibility of damage to blood vessels and nerves exists. There is potential for post-operative complications. A permanent scar is very likely. Choi et al suggests that for pediatric patients, because of their elastic bone structure and thick soft tissue coverage, thin cortical bone and significant premature trabecular bone, no severe impact occurs upon receiving trauma. As such treatment methods can differ between pediatric and adult patients. They suggest that because most of the growing crown of the permanent tooth has not yet been completed, the ratio of bone tissue to the tooth is relatively low. In addition, in developing dentition, the teeth are in different stages of formation and maturation; they can also easily be in the line fracture which in turn can cause delayed eruption and ankylosis. Intermaxillary fixation of more than 2 weeks is not recommended for these pediatric patients and open reduction is not normally used since it is invasive and there is risk of facial nerve injury. They also report , no significant difference in prognosis is found compared to closed reduction. Conclusion Management of MCF remains a subject area where there is a lack of consensus on whether open reduction or closed reduction method is more suitable. Where there are some convergence of ideas include that intracapsular fractures are better treated closed6, it is better to deal with pediatric patients with closed reduction where practical and that physical therapy post treatment is important. Traditionally, closed reduction techniques were more prevalent; however with enhancement in tools available for surgical intervention, open reduction techniques are being practised more, with the added benefit of evidence based studies on such techniques becoming more available.
